Heather Peters, FNP-C,CF-L2 is a Family Nurse Practitioner with a passion for helping people thrive—whether they’re managing chronic illness or just trying to stay well.
After earning her nursing degrees from Wilkes Community College, Winston-Salem State, and East Tennessee State, Heather worked in both hospital and school settings before launching her own health and wellness practice with dietitian Maggi Birdsell.
Her mission? To help patients ditch chronic conditions, stay healthy for the long haul, and feel empowered every step of the way.
Heather lives in Crumpler, NC, with her husband Marty, their kids Harper and Hadley, and a lively crew of pets including three dogs, a cat, and a rabbit. When she’s not seeing patients, you’ll find her lifting weights at CrossFit, treasure hunting at flea markets, or planning her next family adventure.
Maggi Birdsell, RD, LDN, CDE, CF-L2 is a dedicated Registered Dietitian and Certified Diabetes Educator with a deep-rooted passion for helping others use food as a powerful tool for wellness. While she didn’t set out to pursue a career in nutrition, she discovered her calling during her sophomore year of college—and never looked back.
Maggi earned her Bachelor of Science in Nutrition and Dietetics from the University of Delaware. After obtaining her R.D. credentials, she moved to the mountains of western North Carolina in 2009, where she spent nine impactful years serving her community through AppHealthCare.
Driven by a desire to make personalized nutrition care more accessible, Maggi co-founded a private practice alongside Heather Peters. Her mission is simple but powerful: to equip individuals with the knowledge and confidence to take charge of their health and use nutrition as medicine.
Maggi lives in Jefferson, NC, with her husband Travis, their two daughters Alice and Annabelle, their dog Harper, a turtle named Rosie, and a lively mix of chickens and ducks. Outside of work, you’ll find her cooking from scratch, exploring mountain trails, gardening, staying active, and giving back through local community organizations.
Optimal health is not achieved through one approach alone—it is built through the balance of medical care, nutrition, and movement. At Elevation Health, that balance becomes a personalized, achievable path forward.
After 32 years in emergency medicine, including the last 8 years at Ashe Memorial Hospital, Dr. Fleming has seen firsthand the consequences of a reactive healthcare system—one that often addresses illness only after it has already taken hold. Day after day, he treated patients suffering from preventable and often misunderstood conditions rooted in the natural changes that occur with aging—particularly hormonal imbalance.
Driven by a desire to shift from crisis care to true health optimization, Dr. Fleming is now focused on helping patients feel, function, and live better—before disease progresses. He is especially passionate about restoring attention to hormone health for both men and women, an area he believes has been overlooked or minimized for far too long. Symptoms such as fatigue, brain fog, mood changes, poor sleep, decreased strength, and sexual health concerns are often dismissed as “normal aging”—but they don’t have to be.
At Elevation Health, Dr. Fleming works alongside Family Nurse Practitioner Heather Peters and Registered Dietitian Maggi Birdsell to provide a fully integrated approach to care. By combining precision medical care, targeted nutrition, and individualized fitness, patients are supported from every angle—creating sustainable, meaningful change.
This model is not about quick fixes—it’s about partnership. While no provider can do the work for you, the right team can guide, support, and equip you with the tools to take control of your health.
If you’re ready to stop accepting how you feel and start optimizing how you live, we’re here to help you take that first step.

I’ve always had a heart for helping others, and that passion led me to a career in healthcare. I began my journey at Lincare, where one-on-one patient care sparked my love for medicine.
After earning my Phlebotomy certification through Wilkes Community College, I worked as a Medical Office Assistant and Phlebotomist at Prime Medical Healthcare in Boone, NC, for seven years.
I later joined Mount Jefferson Family Medicine as Lab Director and MOA, where I discovered just how meaningful patient relationships could be. What started as a job quickly became a calling—to educate, support, and empower patients to take control of their health.
Now, I’m proud to be part of a team that values personalized care, and I’m grateful for every opportunity to make a difference.
